The Integrated Comprehensive Care (ICC) program is an evidence-based model of Bundled Care that supports One Team, One Record, One Number to Call, 24/7.

ICC YourCare+ is an online support platform powered by YourCare+. The platform provides ICC patients and families with information, tools, and self-assessments to support smooth care transitions and improve care in the home.

COVIDCare@Home supports COVID-19 positive patients with 24/7 access to one integrated team across multiple sectors, alongside partners in primary care and public health.

This forthcoming report showcases the collective strengths of the Connected Health Hamilton collaborative, highlighting their coordinated efforts to meet the needs of COVID positive patients and at-risk populations in the Greater Hamilton area.

The CIC Maturity Model is a practical hands-on resource for organizations of any size, at any point in their implementation journey. It provides an engaging way to self-assess and determine next steps and uncover ‘accelerators’ to move towards maturity.

The objective of this work is to implement and evaluate a discharge AI process whereby patients with health instability are identified, the risk is communicated, and resources that facilitate the transition to self-management are provided.
