CIC’s Community of Practice (CoP) is a collision space to share and accelerate innovative work in Integrated Care. The CoP is a vehicle to share knowledge, drive implementation, spread, and scale of evidenced-based models of care. Our CoP collaborates on projects to generate new knowledge and develop lasting relationships with partners. Our overall goal for the CoP is to foster innovation and to improve quality of care.

What is a CoP?

A CoP is “a group of people who share a concern, a set of problems, or a passion about a topic, and who deepen their knowledge and expertise in this area by interacting on an ongoing basis.” The method of interaction does not define the CoP. The members of a CoP can share and build their knowledge by communicating online, in person, through webinars or teleconferences, or through any other means that works for the participants. (HQO)

How are CoP sessions delivered?

At this time, CoP sessions are mainly delivered live (either virtual or in-person). Sessions take on a ‘case-based’ learning approach, and are highly interactive. New ways of delivering sessions will be explored as the CIC CoP matures and expands.

What kind of topics will be discussed in CoP sessions?

Potential topics include: breaking down silos between disciplines and sectors, change management, performance evaluation, partnering with non-traditional partners.
