AMS Healthcare Fellowship in Compassion and Artificial Intelligence

The AMS Healthcare Fellowship in Compassion and Artificial Intelligence (AI) is a funding opportunity awarded to applicants who explore how digital health and AI can provide innovative and compassionate care solutions.

Supported by AMS Healthcare, Dr. Connie Schumacher of Brock University is evaluating the pilot of the YourCare+ Check-Up (CU) Assessment. This project is in collaboration with St. Joseph’s Home Care and the Centre for Integrated Care, and will examine the effectiveness of the CU assessment as part of the Integrated Comprehensive Care (ICC) program discharge process.

The YourCare+ Check-Up is a digitally offered, validated interRAI self-report assessment. The CU summary report identifies patient strengths and areas of concern based on built-in assessment algorithms, and provides custom YourCare+ resources that correspond with needs.  Patients and caregivers can share the CU report with care providers to communicate health and social needs.

This study will analyze and examine the YourCare+ Check-Up Assessment report to determine:
1) if the risk is communicated in a compassionate and caring manner, and
2) if the recommended resources align with identified needs.

The objective of this work is to implement and evaluate a discharge AI process whereby patients with health instability are identified, the risk is communicated, and resources that facilitate the transition to self-management are provided.

What is the YourCare+?

YourCare+ is a digital education platform for caregivers and people receiving care. The platform provides practical tools, information, and self-assessments designed to improve system navigation, promote caregiver wellness, provide essential knowledge and skills to manage chronic disease symptoms, and connect needs with services.

A custom version of YourCare+, ICC YourCare+, has been developed to support Integrated Comprehensive Care (ICC) patients and caregivers during care transitions. The ICC YourCare+ platform will be used for AMS Fellowship study.

What is the YourCare+ Check-Up Assessment?

The YourCare+ Check-Up Assessment helps caregivers and people receiving care identify and manage complex health conditions using a validated interRAI self-report assessment. The Check-Up creates a summary health report using standardized language that can be printed and shared with care providers to efficiently communicate health and social needs. The Check-Up has most recently been used locally in home and community care in response to the COVID-19 pandemic.
