ICC YourCare+

ICC YourCare+ is an online platform supporting care throughout the Integrated Comprehensive Care (ICC) program journey. The website targets patients, caregivers and family members to improve knowledge and skills needed for effective chronic disease management in the home.  

The website was adapted from YourCare+ (www.yourcareplus.ca), and co-designed with older adults, caregivers, health professionals, and an interdisciplinary research team. The website offers comprehensive self-assessments, actionable tools, and information to support ICC program navigation, caregiver health and well-being, managing cardiorespiratory symptoms, and communicating effectively with healthcare professionals.

YourCare+ self-assessment solutions are being tested and evaluated by Dr. Connie Schumacher of Brock University as part of an AMS Healthcare Fellowship in Compassion and Artificial Intelligence. This project is in collaboration with St. Joseph’s Home Care and the Centre for Integrated Care to examine the effectiveness of client self-assessment as part of the Integrated Comprehensive Care (ICC) program discharge process. Individuals and caregivers who aren’t enrolled in the ICC program are able to access resources through the publicly available platform (www.yourcareplus.ca).
