Implementing and sustaining integrated models of care requires people and teams, a common vision, time, and trust. The Centre for Integrated Care offers a variety of services from skilled practice coaches to support you and your network in meeting your goals for successfully adopting new integrated care models.


CIC’s Coaching & Advisory services provide:

  • A path forward for individual programs or broader Health Teams by guiding the completion of a Readiness Assessment and facilitating development of integrated care program essentials. These may include establishing shared goals, common vision and strategy, joint funding and planning, evaluation and quality improvement.

  • Resource secondment and 24/7  ‘Phone a Friend’ to support development and implementation of sustainable integrated care models within one or more clinical pathways across a defined patient population.

  • Strategic partnerships to build knowledge and capacity around Integrated Care, enabling the sustainable and scalable adoption of evidenced based approaches to integrated care across providers the region.

  • Development of Lead Agency Model to enable impactful change in partnership with Home and Community Care sector:

    • Establishing and maintaining Integrated Care Model

    • Management & Oversight of front line care delivery

    • Provision of technology, integrated community health record

    • Home and community care service provision

Can you give a specific example of how you’ve helped other organizations?

An organization came to us with the goal of improving an existing @home program. Our team worked with the provider and broader Ontario Health Team to complete a readiness assessment, building awareness, knowledge, and capacity around Integrated Care within the team. Through resource secondment, we supported the adoption of best practices established via the ICC model. This process allowed us to improve and refine the existing model and support subsequent roll-out. A sustained roll-out was supported through the collaboration with the provider to explore assessment tools and guide development of a robust evaluation program.

What is the “Phone a Friend” Service?

If we have the opportunity to work with you, we are here for you.  Help is only one call away. The “Phone a Friend” service is available 24/7 for healthcare organizations to contact an expert in integrated care, just like you would phone a friend, during the development and implementation stages of an integrated care program.  Having a difficult conversation with a stakeholder and want a second opinion? Immediate assistance can be provided, and further action can be organized.
