Integrated Care Discovery Sessions

At the Centre for Integrated Care, we are passionate about learning and sharing knowledge, experience and strategies with like minded individuals whose mission is to integrate systems and remove barriers to advance people-centred care. You are invited to attend and inspire, collaborate, and explore implementation strategies for operationalizing Models of Integrated Care with your own local environment.

Examples of Discovery Session Goals:

  • Learn about the Integrated Comprehensive Care (ICC) Model

  • Understand approaches and benefits of co-designing and implementing an Integrated Service Delivery Model through patient and caregiver, staff, and community engagement

  • Explore how to build and manage and Integrated Funding Model (IFM)

  • Learn about experiences working with home and community care as partners, and St. Joseph’s Home Care “Lead Agency” model

  • Share your specific questions and engage in discussion with CIC team

Integrated Care Discovery Sessions provide participants with the opportunity to learn about existing evidenced based models of integrated care (e.g. ICC), share and explore CICs Innovation Incubator and Acceleration Hub activities.

When are sessions held and how do I register?

Sessions are arranged quarterly or more frequently based on demand. They typically run 3 hours in length with the option to attend virtually or in person (space permitting).
