Mature integrated health systems have transitioned from condition-based care targeted at specific patient diagnostic groups to comprehensive, community-wide integrated health and social care that supports improved population-level health. However, to achieve maturity, a greater intensity of coordination, collaboration, partnerships and co-design are required.

The Centre for Integrated Care (CIC) is an innovation centre committed to research, education and evaluation to support the advancement of integrated care across Canada. Based on our extensive experience working with patients and providers to support the development, adoption and evaluation of models of integrated care, we are developing a maturity model for integrated care. As a practical hands-on resource for organizations of any size, at any point in their implementation journey, the model’s domains, related score cards and key enablers will provide an engaging way to self-assess and determine next steps and uncover ‘accelerators’ to move towards maturity.

The maturity model can be used as a practical resource for anyone involved in integrated care, including: decision-makers, stakeholders from health care organizations, agencies, government, policy-makers, patient and family partners and any other participants interested in the design and implementation of integrated care models.

What is a maturity model?

Maturity models are used as quality improvement tools to assess a person or organization’s effectiveness and help determine what supports are needed to improve performance. Maturity stages describe increasing levels of performance (the higher, the closer to being an integrated care delivery system)

How will the CIC maturity model help advance integrated are?

By creating a maturity model:

  • We develop a common language to describe levels of performance and what is needed to reach the next level

  • Organizations can assess their own level of performance and identify requirements to reach the next level

Who is this tool for?

  • Health and social care system planners — To assess and understand the status of the system and determine steps necessary to move towards the next levels of maturity in integration across the health and social care system

  • Organization/program leaders/providers — To assess and understand the status of the organization/program and determine steps necessary to move towards the next levels of maturity in integration for their organization/program

  • Patients and families — To understand how well the system is working for them and understand how the system should actually work (to support self-advocacy and engage in system design)
