The Centre’s current priorities are to advance and innovate integrated health care in the following areas:  

Care Beyond Boundaries

People want to age in non-institutional settings. The Centre for Integrated Care wants to pioneer new, flexible public and private care models to support aging in place for people across care settings and informed by social determinants of health. 

Empowered System Navigation & Care Options

Caregivers (e.g., family members, friends, neighbours) take on a bulk of care responsibilities for individuals requiring ADL and IADL support in the home. Yet, there are insufficient navigation solutions built for them. CIC is leading new health care navigation solutions to support caregivers in providing better and more sustainable care. 

Data Platforms for Learning Health Systems 

Learning health systems are predicated on the use of high-quality, real-time data. The Centre for Integrated Care is leading new EMR platforms with health service providers that are the backbone for emerging learning health systems.
