Meet the Team
Connie Schumacher, PhD
Dr. Connie Schumacher is an Assistant Professor of Nursing at Brock University. She is a registered nurse with expertise in emergency avoidance and transitions from hospital to home. Connie has 20 years of clinical experience in Cardiac Care and Emergency, and 5 years in Community nursing. Connie was the Lead Clinical Coordinator for the DIVERT-CARE, a multi-Provincial RCT in home care. She is the clinical content lead for YourCare+ and has an interest in senior’s care, chronic disease management, interRAI applications, and promoting evidence-based care in the home care sector. Connie is a recipient of this year's AMS Fellowship in Compassion and Artificial Intelligence.
