YourCare+ is an online platform that helps caregivers care for themselves and others by providing tools to use in the home, to send to a family member or friend, or share with health care professionals.
The Integrated Comprehensive Care (ICC) program is an evidence-based model of Bundled Care that supports One Team, One Record, One Number to Call, 24/7.
This model of care supports COVID-19 positive patients with 24/7 access to one integrated team across multiple sectors, alongside partners in primary care and public health.
