Michelle joins the Centre for Integrated Care as a Care Design Leader. Michelle is a Registered nurse with over 20 years of front line and management experience in acute care, long term care and community care nursing. In her current role is Director of Clinical Programs at St Joseph’s Home Care, she is responsible for leading two large community-based programs, the Visiting Nurse and Integrated Comprehensive Care (ICC) Programs. She has successfully expanded the ICCP across several patient populations and continues to explore and evaluate innovative models of care, including virtual. She works with a variety of professionals including many different health disciplines along with several external partners to ensure the delivery of high quality care.
What motivates your work in integrated care?
I believe integrated care models help simplify the patient experience and encourages health care partnerships. Having 1 team, working together who all have access to their health record, and 1 number to call is very reassuring to patients and their families
