Health & Housing in the Community
Housing is a fundamental element of a person’s life. Housing encompasses living environments and living arrangements, affordability, stability, access to resources and services, as well as social supports ,and community belonging. Housing is more than a place to live. It is a place that shapes stability, safety, and access to care.
Lead by Centre for Integrated Care’s Research Scientist Dr. Chi-Ling Joanna Sinn, our research is conducted to explore integrated care models that work for all individuals across the housing continuum. This includes
Coordinating health and service delivery within congregate living settings and at the neighbourhood level
Strengthening links with community resources
Eliminating gaps in care transitions
Advocating for more equitable housing and health policies
