About Centre for Integrated Care
Our History
The Centre for Integrated Care builds on the renowned St. Joseph’s Health System Integrated Comprehensive Care (ICC) program pioneered in the region in 2012 and expanded to other regions in Ontario. It also builds on St. Joseph’s Health System partners that reflects the continuum of health care services, including: acute inpatient care, inpatient and outpatient mental health, complex continuing care, residential palliative care, long-term care, seniors housing, home care, and community support services.
The Centre for Integrated Care was first formed in 2018 with the initial purpose of formalizing, sharing and expanding the expertise acquired from the development, spread and scale of the renowned St. Joseph’s Health System Integrated Comprehensive Care (ICC). Our research unit began in late 2019 with the hiring of our Director, Andrew Costa. Our Associate Director, Dr. Justin Lee, began in late 2020.
Today, the Centre stands as a hub for innovation and collaboration, turning evidence into action and supporting health system partners in delivering more connected, person-centred care.
