Clinical Care Optimization for Older Adults

Each aging experience is unique, shaped by its own complexities and circumstances. Providing effective clinical care requires more than traditional medical interventions,it calls for an approach that recognizes the whole person. Clinical care optimization embraces this by delivering care that is holistic, individualized, and patient-centred. 

 

Clinical care optimization for older adults focuses on developing innovative strategies to support an integrated clinical care approach for older adults. Primary research is conducted to better understand the comprehensive health and social needs of patients living with frailty and knowledge translation interventions to facilitate senior-friendly care across the care continuum. Current projects include the development of digital EMR and care interventions to support comprehensive frailty care, medication optimization during transitions of care, and delirium- and dementia-friendly hospital care.
