      <image:caption>Joanna Sinn is a Research Scientist with St. Joseph’s Centre for Integrated Care and part of the BDG scientific leadership team. Dr. Sinn is a health services epidemiologist and program evaluator specialising in integrated health systems research, implementation, and evaluation. She conducts collaborative community-based research employing mixed methods, participatory approaches, and broad engagement strategies across the knowledge-to-action cycle.   Dr. Sinn is passionate about community health priorities, especially those at the intersection of individual and community health as well as social, economic, and physical environments. Dr. Sinn’s current research focuses on home and community care and housing with supports. Her approach to research brings system partners together with system data to identify needs, develop new models of care, and evaluate meaningful outcomes. She also frequently draws upon quality improvement principles to translate research outputs into better processes, programs, and policies. Her research focuses on the development, implementation, and evaluation of strategies to improve equitable access to community-based health care services for older adults. Sophie recently completed a CIHR Health System Impact Fellowship jointly with Women’s College Hospital Institute for Health System Solutions and Virtual Care and McMaster University. She focused broadly on the intersection of home care and community support services, and equitable access to these services by underserved populations, particularly in light of the switch to virtual services as a result of public health measures during the COVID-19 pandemic. She holds a PhD from the School of Public Health and Health Systems at the University of Waterloo and has received national and provincial awards and funding for her work.       <image:caption>Justin Lee is an Assistant Professor in the Department of Medicine at McMaster University. He is actively involved with the clinical care of patients as a staff geriatrician and internist at St. Joseph’s Healthcare Hamilton and Hamilton Health Sciences. He has expertise in clinical pharmacology and the care of older adults including those living with multiple co-morbidities and frailty. This stems from his training as both a clinical pharmacist with a hospital residency at the University of Toronto and a specialist physician in Geriatric Medicine and Internal Medicine at McMaster University. Justin serves as an Associate Research Director at the St. Joseph’s Health System Centre for Integrated Care and a Clinician Scientist at the GERAS Centre for Aging Research. He completed a PhD in Health Research Methods at McMaster University focusing on clinical epidemiology and health services research. He has a special interest in better understanding patients with high healthcare needs or costs, improving the transitional care period from hospital to home and optimizing medication use in older adults. His program of research is focused on developing innovative strategies to improve health systems and achieve the Quadruple Aim of better population health, improved patient and clinician experience and reduced healthcare costs. Justin has received several provincial and national awards for his research, leadership and academic achievements including a Canadian Institutes of Health Research (CIHR) Fellowship and Silver Award of Excellence, the David Sackett Scholarship, the Canadian Geriatrics Society Edmund Cowdry Award and studentship awards from the CIHR Drug Safety and Effectiveness Training program and the Ontario Drug Policy Research Network.   What motivates your work in integrated care? “In my experience both as a clinician and as a caregiver for my family, I routinely witness the impact and hear the challenges faced by patients and families as they seek care in our complex healthcare system… and I believe we can do better. I have seen the possibilities when the care of my patients is more patient-centred and providers are better connected and able to work as a team across the continuum of care.”</image:caption>

      <image:caption>Lindsay joins St. Joseph’s Health System Centre for Integrated Care as a Research Manager where she supports a variety of research initiatives within the Centre’s research portfolio. Currently, Lindsay is working on multi-year study aimed at identifying components of a sustainable integrated primary care model for residential care facilities. Other initiatives she supports include self-service navigation for caregivers and individuals living in home and community care settings, and practical assessments aimed at building capacity and advancing maturity within integrated care programs and organizations. Lindsay’s interests include the access and equity of care support and resources, and people-centred care.  What motivates your work in integrated care? My mom was diagnosed with Multiple Sclerosis when I was three years old. As her care needs became increasingly complex, our family navigated Emergency Department admissions, readmissions, and providing complex care at home. With time, we learned how to support my mom’s evolving care needs, but not without experiencing the impact of fragmented care delivery on her and my family’s quality of life. I am grateful to work in integrated care because of the opportunities CIC, alongside our partners, have to innovate and advance connected and people-centred care.</image:caption>

As Managing Director of the OnSPARK LTC Data Platform, Gioia serves as the central administrative and delivery leader, overseeing internal operations such as human resources, budget and financial management, governance coordination, stakeholder liaison, and executive support for the platform.  Prior to joining CIC, Gioia spent 25 years at McMaster University’s Centre for Health Economics and Policy Analysis (CHEPA), where she served as Senior Research Coordinator and Research Associate in Health Economics. Her work spanned program management, health economics and policy research, and operational leadership. She has extensive experience managing large-scale research programs, conducting data analysis, developing and monitoring budgets, coordinating deliverables, and fostering collaboration among academic, government, and community partners.   Gioia holds an Honours Bachelor’s degree in Economics and Commerce from the Università degli Studi di Perugia, Italy, and a Master of Arts in Economics from McMaster University. At CIC, Michaella has been working with the OnSPARK team to establish and build the health human resources (otherwise known as Health Workforce) dataset for long-term care. The OnSPARK data platform provides a previously unrealized opportunity for evaluation, workforce monitoring, and planning tool for the long-term care sector.  Using these data along with qualitative approaches, she investigates how staffing models can be improved to meet the care and social needs of residents, and the needs of staff in establishing a psychosocially healthy workplace.       <image:caption>Samaneh Farahani is a Research Associate at the Centre for Integrated Care and a PhD candidate in Health Policy at McMaster University. She also holds a Master’s degree in Health Administration (Community Care) from Toronto Metropolitan University.  She has collaborated with long-term care and community organizations to advance innovative models for aging in place and homecare and has conducted research on digital inclusion and community integration among migrant populations. Samaneh previously co-led a project at the Community Services focused on refining the organization’s client experience survey for its Health Services portfolio, developing an evidence-based, culturally responsive framework to improve data quality and strengthen quality improvement processes.  Samaneh’s research interests include aging and elderly care, workforce optimization, health services research, and healthcare quality, with a particular focus on using population-level data and policy analysis to improve care coordination and system integration.  Her current project, Changing Resident Composition in Ontario’s Long-Term Care Homes (2017–2025), examines longitudinal shifts in the demographic and clinical profiles of Ontario’s long-term care residents over time.</image:caption>

Her research focuses on the nutritional determinants of health, with a particular emphasis on how long-term dietary behaviors influence population health and cognitive decline among older adults.  Mahtab’s experiences as a Registered Dietitian inspired her to look beyond individual food choices and focus on the long-term habits and environments that shape people’s health. She believes that many individuals know what constitutes a healthy diet, yet face environmental and social barriers—such as family eating patterns, food accessibility, equity, and the influence of meal companions—that limit their ability to make sustainable changes. Mahtab is passionate about people-centred care that integrates patients into the design of their dietary plans, recognizing that eating should remain an enjoyable and empowering part of life.
